Herbert drove D6 into action on 15 September 1916. The tank almost breached the defences of Gueudecourt but was destroyed by enemy artillery. Herbert was awarded the MM for his actions on that day. He was subsequently commissioned into the Army Sevice Corps on 4 March 1917 but was drowned less than six weeks later when the ship on which he was deployed to Egypt was torpedoed.
